Any employee who incurs a work related incident may elect to complete an Employee Incident Report form.  If medical treatment is necessary, an administrator or designated administrative secretary may complete an authorization for treatment form which will enable the employee to be seen by a doctor.

 

Employee's who have been involved in a work related incident may review the Workers' Compensation Helpful Info memo form for instructions and contact information.
