Procedures for Reporting a Work Related Incident or Injury

Employees of the Agency who suffer an injury/accident while on the job must file a report with their Supervisor immediately - not to exceed 24 hours.

Procedures for Reporting a Work-Related Incident/Injury:

1. You must immediately report the Work-Related Incident to your Supervisor not to exceed 24 hours. 
2.  Your Supervisor will complete a Supervisor's Report of Incident and will forward a copy of the report to the Human Resources Department. 
3. If needed, an Authorization for Treatment will be provided to you. 
4.  Your Supervisor will direct you to the Agency's Board Approved Occupational Health Clinic for medical treatment, if necessary. 
5.  Injuries requiring immediate emergency attention shall be referred to nearest hospital emergency room, or, call 911. 

Post Treatment Procedures:

If you are released to return to regular duty work, you must provide Human Resources with a copy of the release prior to returning to work 
 ● If you are released to return to work with restrictions, you must immediately notify Human Resources.
PLEASE NOTE: You may not return to work until you receive confirmation and/or approval from Human Resources. 
 ● If you are released with restrictions, restricted, light duty work may be offered by the Agency, prior to your return to work. 
 ●  Never place yourself or co-workers at risk by performing duties that you have been restricted from performing.
